Breast milk smells soapy: what the guidelines say about lipase

A soapy, metallic or fishy smell in thawed milk is something the guidelines actually describe, and one of them describes it in detail. ABM Protocol #8 says the odour of refrigerated and frozen milk comes from lipase-mediated fat breakdown, and that there is no evidence infants often reject milk because of it. What no guideline publishes is a smell test: nothing here tells you a bag has spoiled, and nothing here replaces the date. What the ABM protocol actually says about the smell
Protocol #8 carries a numbered section titled "Smell of stored milk". These are its sentences, quoted from the protocol on 2026-09-08:
"Refrigerated and frozen human milk may have an odor different from fresh milk due to lipase-mediated triglyceride breakdown, releasing fatty acids. The odor likely comes from oxidation of these fatty acids."
"This lipolysis process has antimicrobial effects preventing the growth of microorganisms in thawed refrigerated milk."
"There is no evidence to suggest that infants often reject human milk due to this odor." "Many foods that humans eat, such as eggs, cheese, and fish, have an unpleasant odor that does not affect taste."

Read those four sentences together and they do a specific, limited job. They explain where the smell comes from, they note that the process itself has an antimicrobial effect in thawed refrigerated milk, and they push back on the assumption that the smell means a baby will refuse the bottle. They do not say the milk is fine, they do not say it is not, and they give you nothing to sniff for. That is the honest boundary of the source.
Does a soapy smell mean the milk is spoiled?
The question a smell can answer is "has something changed", and the answer is yes: freezing changes stored milk, and that is documented. The question a smell cannot answer is "is this bag inside its window", because none of the guidelines this site cites publishes a smell, taste or appearance test. What all of them publish is a duration.

The scalding question, and why this page does not print a method
This is the part most searches are really about, and it is the part where the two sources part company.
- KellyMom publishes a scalding method for newly expressed milk, with temperatures, on its lipase page. The link is in the sources below.
- ABM Protocol #8 says, in the same section quoted above: "Heating milk to above 40 °C to deactivate lipase is not advised because this may destroy many of the immunologically active factors in human milk."
Those are not two ways of saying the same thing. One describes a technique; the other advises against the goal of the technique, for a stated reason. A landing page is not the place to settle that, so this one does not reprint the method. The link is there, the objection is there in the protocol's own words, and the decision belongs to you and an IBCLC who can look at your milk, your baby and your supply together.
If you take one thing from this section, make it the shape of the disagreement: the objection is not that scalding fails to work on the smell, it is about what else the heat does to the milk.
Does a colder freezer help?
The protocol mentions this too, and the number in it is easy to misread: "One study demonstrated that freezing human milk to −80 °C (−112 °F) leads to less change in smell as compared to conventional freezing to −19 °C." That is a laboratory freezer. A domestic chest freezer does not reach −80 °C, and no guideline publishes a home-freezer temperature at which the smell changes less.

What you can do that costs nothing to say
Two of these are practical decisions about your own stash rather than clinical ones, which is why they are the only suggestions on this page:
- Test one bag before deciding about a hundred. Thaw a single bag that is inside its estimated window, offer it, and see. The ABM's own line is that there is no evidence infants often reject milk over this odour, and one bag answers the question for your baby specifically.
- Write the smell down, next to that bag. "Smelled soapy, she took it" or "refused it" turns a vague worry into a pattern you can actually see, and it tells you whether the change is tied to a date, a freezer or nothing at all.
- Do not discard a stash on a hunch, and do not keep a bag past its window on one either. The date is the rule in both directions.
